This is the 2010 membership form for new players. Please download and bring it along to the registration evening on the 23rd April. Please download and complete the Youth Player Profile form, bring the players birth certificate and NHS card for an offical to see it to complete the form.

Please download and complete the whole Youth Player Profile form, bring the players birth certificate and NHS card, or copies. The manager of the youth team your child is playing can confirm the numbers on the form.
